Telehealth Consent

Purpose of this Consent
This Consent explains the use of telehealth in connection with healthcare services provided to you by independently licensed healthcare professionals (each, a “Provider”), including nurse practitioners, physician assistants, and physicians, who are solely responsible for all clinical decision-making and who practice under a Medical Director arrangement where required by law.
Informed Consent Regarding Telehealth
What Telehealth Is: Telehealth involves the delivery of healthcare services using electronic communications when you and your Provider are not in the same physical location and may include secure messaging, questionnaires, audio or video consultations, review of medical information, and follow-up care. Telehealth services are provided only where permitted by law and are based on your physical location at the time services are rendered. Your Provider will determine, in their sole clinical discretion, whether telehealth is appropriate for your condition. Telehealth has inherent limitations, including the inability to perform certain physical examinations or diagnostic testing. Alternative methods of care, including in-person services, may be available and can be discussed with your Provider.
Anticipated Benefits
The use of telehealth may provide certain non-clinical benefits, including increased access to healthcare services, the ability to communicate with a Provider without an inperson visit, and improved convenience in scheduling and follow-up care. No specific medical outcomes, diagnoses, treatments, or results are guaranteed.
Potential Risks and Limitations
Telehealth involves the use of electronic communications and technology and, as a result, may be subject to limitations or interruptions that could affect the timing or scope of care. Because telehealth does not involve an in-person examination, there may be limitations in a Provider’s ability to fully assess certain conditions. Telehealth may not be appropriate for all medical concerns. Your Provider will determine, in their sole clinical discretion, whether telehealth is appropriate and may require in-person care, referral to another provider, or emergency services when clinically indicated.
Emergencies and Urgent Care
Telehealth services are not appropriate for emergency situations. If you believe you are experiencing a medical emergency or a life-threatening condition, you must seek immediate care by calling 911 or going to the nearest emergency department. If you are experiencing thoughts of self-harm or suicide, call 911 or the 988 Suicide & Crisis Lifeline immediately. Providers may not be able to review or respond to communications submitted through the platform in real time. Telehealth should not be relied upon for urgent or emergent care.
Data Privacy and Protection
Your medical records are created, maintained, and controlled by your Provider in accordance with applicable federal and state laws, including the Health Insurance Portability and Accountability Act (HIPAA). Regenyx provides non-clinical technology and administrative support services and may process certain information on behalf of Providers in accordance with applicable law and contractual obligations. Regenyx does not practice medicine and does not control clinical records.While reasonable administrative, technical, and physical safeguards are used to support the platform, no electronic system can guarantee absolute security, particularly when communications involve third-party services such as email, mobile networks, or internet service providers. Use and disclosure of your protected health information are governed by your Provider’s Notice of Privacy Practices.
Laboratory Testing
Your Provider may determine that laboratory testing is medically appropriate as part of your care. Any laboratory tests are ordered by your Provider in their sole clinical discretion. Laboratory testing services are performed by independent third-party laboratories that are not operated or controlled by Regenyx. Regenyx may facilitate non-clinical logistics or technology related to testing but does not perform, supervise, or interpret laboratory testing. Laboratory results may be delayed, incomplete, or inconclusive, and no specific results or outcomes are guaranteed.
Pharmacy Services
If a prescription is issued, it is issued solely by your Provider based on their independent clinical judgment. You may choose any pharmacy of your choice to fill your prescription. However, certain medications, including compounded medications, may only be available through specific pharmacies capable of preparing or dispensing such medications. If you elect to use a pharmacy accessible through the platform, prescriptions may be transmitted electronically to that pharmacy at your request for fulfillment. Regenyx maintains relationships with independent third-party pharmacies to facilitate access to prescribed medications as a matter of administrative convenience. Pharmacy services are provided solely by independent pharmacies that are not operated or controlled by Regenyx. Regenyx does not dispense medications and does not influence prescribing decisions. If you prefer not to use a pharmacy accessible through the platform, you may request that your prescription be sent to another pharmacy of your choosing. Regenyx and your Provider do not guarantee that an alternative pharmacy will be able to fill the prescription. All services are offered on a private-pay basis only. Regenyx and the Providers do not submit claims to insurance, Medicare, Medicaid, or other government healthcare programs.Medication availability, fulfillment timing, and pricing are determined by the dispensing pharmacy and are not guaranteed.

The federal Physician Payments Sunshine Act requires certain financial relationships between healthcare providers and manufacturers of drugs, medical devices, and biologics to be publicly disclosed. Financial Disclosures
Regenyx maintains contractual relationships solely for non-clinical administrative, technology, and operational support services, which are provided at fair market value and are not determined by the volume or value of any clinical services, prescriptions, or referrals. These arrangements do not influence clinical decision-making. You are free to seek care from any healthcare provider not affiliated with Regenyx. However, Regenyx Services are designed to support care delivered by Providers who utilize the Regenyx platform, and Regenyx does not guarantee the availability or functionality of services for care delivered outside of the platform.

By accepting this Consent, you acknowledge and agree that:
Healthcare services are provided via telehealth by independently licensed Providers.
Providers retain sole authority over diagnosis, treatment, and prescribing decisions.
Telehealth has limitations and may not be appropriate in all cases.
Your Provider determines, in their sole clinical discretion, whether telehealth is appropriate for your care.
You may withdraw consent to telehealth at any time by providing written notice to your Provider.
You are responsible for providing accurate, complete, and truthful information relevant to your care.
Services are offered on a private-pay basis only and are not submitted for reimbursement to insurance, Medicare, Medicaid, or other government healthcare programs.
